Blue Dental Choice PPO Plans

Monthly Pricing is based on age and it’s per person. There are no Contracts and no Enrollment Fees on the PPO Plans.

0-18: $36.04

19-64: $34.47

65+: $52.77

FCL is a Dental Carrier Florida Blue owns that manages their Dental Benefits. They offer two different types of PPO Dental Policies, BlueDental Copayment & BlueDental Choice. The BlueDental Choice Plans work on a coinsurance schedule, while the BlueDental Copayment option works on a copayment schedule.

Dental Plan Applications with FCL must be submitted by the 15th of the month to be effective for the following month.

If you haven’t had any Dental Coverage in the last 60 days with over a year of coverage, then Adults have a 6-Month Wait Period on Basic & Major Services. Preventative Care (cleanings, x-rays, exams) can be completed immediately after activation of policy.

If you already have a dentist, please click the image below to visit FCL’s Online Provider Directory to see if the location you visit participates in this network. In the Dropdown box labeled “Select Plan Name,” make sure you highlight the BlueDental Choice Network.

If you do not have a dentist, we can send you a list of ones in your area that are Accepting New Patients and participating in the Choice Network once you have purchased this plan.

  • If you enroll into any of these plans directly with Florida Blue or Florida Combined Life, then their Corporate Office will be your Agent of Record and that can NEVER be changed unless you have a lapse in coverage, which is not advised!
