Monthly Premium: $12.99 + $35 Enrollment Fee

Annual Premium: $143.88 + $35 Enrollment Fee

As long as there is no lapse in coverage, you only pay the Enrollment Fee once.

FCL is a Dental Carrier Florida Blue owns that manages their Dental Benefits. They offer one HMO Dental Plan, which is the BlueDental Care FI315.

HMO policies require a General Dentist to be tied to your plan and if you need to see any type of Specialty Dentist, you will need a referral from your General Dentist first.

Dental Plan Applications with FCL must be submitted by the 15th of the month to be effective for the following month.

There is not a 6 month Wait Period for Adults on Basic or Major Services, but it is an Adult ONLY Plan.

This plan does not have a Cap on services like the FCL PPO Plans do.

The BlueDental Care FI315 requires a Year Contract as well as a one-time $35 Enrollment Fee.

It also requires Automatic Payment for the monthly premiums, unless you’d like to pay for it annually which would save you $12 a year.

If you do not have a dentist, we can send you a list of ones in your area that are Accepting New Patients and participating in the Prepaid F Plan Network once you have purchased this plan.
